Esperanza Health Center’s mission is to provide affordable, quality, bilingual primary health care and support services to our patients in Jesus’ name, and to promote the health of the North Philadelphia community we serve. All people in need of health care services are welcome at Esperanza Health Center.
In addition to primary care, our wholistic services address patients’ spiritual, psychological, and social health needs, and are provided without regard to an individual’s ability to pay. To learn more about the many services we offer, click here.
It is our goal to provide holistic care to our patients across multiple settings. In order to continue providing excellent care, Esperanza Health Center is seeking to become a nationally recognized Patient Centered Medical Home (PCMH). By being a “home” for our patients’ medical care, we will be better able to coordinate the services patients need. A partnership develops between the patient, primary care physician, care team, and outside providers which will help encourage patients to take ownership of their own health. The medical home is a way for the patient to be informed and involved in their care. To the best of our ability, we will coordinate the care of our patients even when it is sought outside our doors, i.e. from hospitals or specialists. Esperanza clinicians stay up-to-date with the current science and provide evidence-based care to our patients to give the best care possible.
We seek to be a community health resource, and are committed to partnering with other organizations and churches through initiatives such as our Community Health Promoter program. In addition, we serve as a place of training and education for students in the health professions.
Esperanza Health Center is recognized as a Federally Qualified Health Center (FQHC) by the U.S. Department of Health and Human Services, Health Resources and Services Administration, Bureau of Primary Health Care.
