Become a Patient

Esperanza Health Center is currently accepting new patients who are covered under all Medicaid, CHIP, and Medicaid sponsored Medicare Advantage plans in SE Pennsylvania. We accept the following insurances:

  • Aetna Better Health: Medicaid and CHIP
  • Health Partners Plans: Medicaid, CHIP and Medicare
  • Keystone First and VIP Choice
  • Medicaid
  • Medicare
  • United Health Care Community Plan for Families and Children

We gladly care for uninsured patients on a discounted fee schedule based on household size and income. We also have Ryan White funding to assist HIV+ patients.

Important Information for New Patients

  • All new patients must begin care with a scheduled New Patient Exam appointment. A limited number of New Patient Exams become available on the first Wednesday of each month. Patients are encouraged to call for the first appointment rather than walking in.
  • Once a New Patient Exam has been scheduled, you will be sent registration forms for completion. New patients will bring completed forms to their first appointment, and should plan to arrive 30-45 minutes prior to the scheduled appointment time to review paperwork with receptionist.
  • All patients MUST bring their insurance card and official photo identification to the first appointment.
  • If available, please bring a summary of your medical history, current health status, medications taken, and recent test results or referral documentation.
  • If you do not have insurance, you may apply for assistance in the Uninsured Patient Program. You will need to meet with the Patient Access Liaison (PAL) prior to your first appointment to review your household proof of income and determine eligibility. Proof of your household income may include:

√  Most recent tax return
√  Two recent paycheck stubs for each working family member
√  Letter from your employer stating your income if working under the table
√  SSA, SSI, or Unemployment award letters

  • Patients of Esperanza Health Center will not be denied access to services due to inability to pay.  If you believe that you are unable to pay the nominal fee that is requested by our sliding fee scale, then please ask to speak with the office manager prior to your visit.

Parking/Transportation

  • Daily parking permits are available for free for patients/visitors at the Philadelphia Parking Authority lot at Clearfield & Ruth Streets. 
    • A parking permit for the day of your scheduled visit is required, which must be obtained from Esperanza in advance or on the day of your visit. 
    • Visitors must park in designated “Esperanza” spaces along the Ruth Street perimeter of the lot, and must display the parking permit on the dashboard of their car.
  • Metered parking is also available on Kensington Avenue.
  • Accessible via SEPTA Market-Frankford El (Allegheny Avenue station) and SEPTA Bus Routes 3, 60 & 89.

Esperanza’s new Hunting Park Health and Wellness Center at 4417 N. 6th Street (near the intersection of N. 6th and Cayuga Streets) offers comprehensive primary care services and new wellness programs that are open to Esperanza patients and to the entire community. Programs will include:

  • Group exercise programs for youth and adults,
  • Healthy cooking classes, 
  • Health education classes, and 
  • Community events, with activities for the whole family.

Parking/Transportation

  • Free on-site parking is available to patients and visitors. 
  • The new health and wellness center is located in the Hunting Park community, a short driving distance from the intersections of Roosevelt Blvd. (U.S. Route 1) and N. 5th and N. 9th Streets. 
  • SEPTA bus stops for routes C and 47 are located several blocks’ walking distance from the Hunting Park location.
