Become a Patient

Esperanza Health Center is temporarily unable to accept new adult patients who are covered under Medicaid, Medicare and Medicare Advantage plans in Southeastern Pennsylvania, as well as adult patients who are covered under other insurance carriers.

We are currently accepting new pediatric patients who are covered under all Medicaid, Healthy PA and CHIP plans in Southeastern Pennsylvania

We are also currently accepting new patients of all ages who do not have insurance. For our patients who are uninsured, payment is on a sliding-fee scale based on income.

Important Information for New Patients

  • All new patients begin care with a scheduled physical exam appointment.
  • New patients should plan to arrive 30-45 minutes prior to your scheduled appointment time.
  • Please bring your insurance card and photo identification to your first appointment.
  • If available, bring a summary of your medical history, current health status, medications taken, and recent test results or referral documentation.
  • If you do not have insurance, please bring proof of your family’s income, such as:
√   Most recent tax return
√   Two recent paycheck stubs for each working family member
√   Letter from your employer stating your income
√   SSA, SSI, or Unemployment award letters
  • Patients of Esperanza Health Center will not be denied access to services due to inability to pay.  If you believe that you are unable to pay the nominal fee that is requested by our sliding fee scale, then please ask to speak with the office manager prior to your visit.

Parking/Transportation

  • Daily parking permits are available for free for patients/visitors at the Philadelphia Parking Authority lot at Clearfield & Ruth Streets. 
    • A parking permit for the day of your scheduled visit is required, which must be obtained from Esperanza in advance or on the day of your visit. 
    • Visitors must park in designated “Esperanza” spaces along the Ruth Street perimeter of the lot, and must display the parking permit on the dashboard of their car.
  • Metered parking is also available on Kensington Avenue.
  • Accessible via SEPTA Market-Frankford El (Allegheny Avenue station) and SEPTA Bus Routes 3, 60 & 89.

Esperanza’s new Hunting Park Health and Wellness Center at 4417 N. 6th Street (near the intersection of N. 6th and Cayuga Streets) offers comprehensive primary care services and new wellness programs that are open to Esperanza patients and to the entire community. Programs will include:

  • Group exercise programs for youth and adults,
  • Healthy cooking classes, 
  • Health education classes, and 
  • Community events, with activities for the whole family.

Parking/Transportation

  • Free on-site parking is available to patients and visitors. 
  • The new health and wellness center is located in the Hunting Park community, a short driving distance from the intersections of Roosevelt Blvd. (U.S. Route 1) and N. 5th and N. 9th Streets. 
  • SEPTA bus stops for routes C and 47 are located several blocks’ walking distance from the Hunting Park location.
