Chronic Disease Management
Esperanza takes a comprehensive, team based approach to provide specialized, personal care for each of our patients with diabetes or at risk for developing diabetes.
During “Diabetes Days,” our patients with diabetes meet with our dietitian/diabetes educator and their personal clinician during the same office visit. We also encourage our patients with diabetes to participate in our diabetes classes, available in English and Spanish.
For our patients with asthma, obstructive lung disease, coronary artery disease, and congestive heart failure, Esperanza clinicians are in partnership with specialist consultants from regional academic medical centers who have extensive experience in the management of these illnesses.
In addition, bilingual community health promoters have been trained to come alongside persons in homes, churches and community centers to teach chronic illness self-management principles.
